PERMISSION SLIP

Before you go on a trip with us you need to do some paper work. Nothing too invasive, just a 27 page form that needs to be filled out in triplicate and notarized. Oh, and don't forget your references. You better get started! (Just kidding about the 27 page part!)

YOUTH WORKER APPLICATION

Do you love God? Like students? Think you want to make a difference in a student's life? Youth ministry may be for you! You don't have to be 22 and "super cool" to be a youth worker--we want youth leaders of all ages and stations of life. If you're interested in becoming a youth leader with Common Ground, here's how to get going!

• Check out the information packet!
• Start visiting youth group activities (check with Pastor Amanda first).
• Go through our Sexual Abuse Prevention Training Course. (Required for anyone working with minors at Life Church!)
• Make a commitment to one or more youth group programs.
• Get to know some students and help point them to Jesus!
